  • Khám phá mới giúp giải quyết việc kháng tamoxifen ở bệnh nhân ung thư vú

    Các nhà khoa học trường đại học Cincinnati (UC) ở Ohio, Mỹ đã tìm ra cơ chế tế bào chuyên biệt trong việc kháng lại liệu pháp hormone đối với bệnh nhân ung thư vú dương tính với estrogen.

    Tiến sĩ Xiaoting Zhang và cộng sự đã xác định đồng hoạt hóa thụ thể estrogen chuyên biệt, MED1, có vai trò trung gian trong việc kháng tamoxifen ở bệnh nhân ung thư vú. Nghiên cứu này được đăng trên tạp chí Cancer Research số ra ngày 1/11/2012.

    Theo thống kê của viện ung thư quốc gia Mỹ, gần 227000 phụ nữ được chẩn đoán ung thư vú hàng năm tại Mỹ. Khoảng 75% trong số đó có khối u dương tính với estrogen và đòi hỏi sử dụng liệu pháp hormone như tamoxifen, thuốc này có khả năng can thiệp vào vai trò của estrogen trong việc kích thích tăng trưởng tế bào ung thư vú.

    Mặc dù đã có những tiến bộ trong  liệu pháp hormone nhưng vẫn có khoảng 50% các ca kháng với các loại thuốc này và tái phát ung thư sau khi dùng.

    Estrogen và progesterone có thể kích thích sự tăng trưởng của ung thư vú. Tế bào ung thư vú nhạy cảm với hormone chứa 1 loại protein chuyên biệt được biết như là thụ thể hormone, thụ thể này được kích hoạt khi hormone gắn với thụ thể, dẫn tới sự tăng trưởng của tế bào ung thư.

    Các nghiên cứu được thực hiện trên cả các mô hình tiền lâm sàng và trên các mô ung thư đều cho thấy rằng MED1 đồng kích thích và đồng biểu hiện với HER2. Gene HER2 tăng sự hiện diện trong 20-30% đối với các ca đến giai đoạn xâm lấn và có vai trò trong kháng tomoxifen.

    Sự biểu hiện vượt mức HER2 dẫn tới sự hoạt hóa MED1, trong khi đó sự giảm của MED1 dẫn tới sự ngừng phân chia của những tế bào ung thư được xem là kháng với tamoxifen. Những nghiên cứu sâu hơn về cơ chế chỉ ra rằng sự hoạt hóa HER2 của MED1 dẫn đến sự quy tụ của các yếu tố đồng kích thích thay vì yếu tố đồng ức chế bởi các thụ thể estrogen có gắn tomoxifen.

    Sự tìm ra tác động tương hỗ giữa 2 phân tử này đóng và trò chủ chốt trong việc khám phá ra cơ chế kháng liệu pháp nội tiết ở bệnh nhân ung thư vú đặc biệt là những công bố gần đây cũng chỉ ra sự biểu hiện ở mức độ cao của MED1 có liên quan tới kết quả điều trị kém hiệu quả cũng như mức độ sống sót không cao của các bệnh nhân sau trị liệu.

    Các nhà khoa học này đang sử dụng kĩ thuật nano để tác động trúng đích MED1 để khóa  con đường estrogen và HER2 nhằm vượt qua khả năng kháng liệu pháp nội tiết của tế bào ung thư vú.

  • Tiềm năng của Curcumin trong điều trị ung thư tuyến tiền liệt

    Gần đây các nhà khoa học thuộc đại học Ludwig-Maximilians, Đức  đã chứng minh vai trò của curcumin trong tác dụng ngăn chặn di căn ở bệnh nhân ung thư tuyến tiền liệt . Nghiên cứu này đã được đang trên tạp chí Carcinogenesis 12/10/2012.

    Nghệ ở dạng bột đã được sử dụng hàng trăm năm trước trong y học cổ truyền để điều trị các loại bệnh khác nhau như viêm khớp và giúp làm ngừng các phản ứng viêm. Vào tháng 3/2012, các nhà nghiên cứu thuộc đại học Michigan State, Mỹ đã chứng minh lợi ích của nghệ đối với bệnh nhân bị bệnh Parkinson, ung thư đầu và cổ.

    Ung thư tuyền tiền liệt là một trong những dạng u ác tính xảy ra thông thường nhất ở phương Tây. Bệnh thường được chẩn đoán khá trễ, khi mà khối u đã ở giai đoạn di căn.

    Curcumin trong nghệ là hợp chất polyphenol tạo ra màu sắc vàng đặc trưng. Curcumin có thể dùng trong liệu pháp ngăn chặn và điều trị bệnh, thậm chí đối với bệnh nhân có khối u di căn.

    Trong nghiên cứu này Bachmeier và cộng sự đã chứng minh vai trò của curcumin đối với quá trình di căn ở ung thư tuyến tiền liệt và tìm hiểu cơ chế của quá trình này. Họ quan sát thấy rằng curcumin giúp làm giảm sự biểu hiện của CXCL1 và CXCL2, đây là chất trung gian kích hoạt tiền phản ứng viêm và có vai trò trong kích thích quá trình di căn. Do đó curcumin có thể ngăn chặn các loại ung thư có liên quan đến quá trình viêm như ung thư vú hay ung thư tuyến tiền liệt.

    Bachmeier nói rằng mỗi người có thể sử dụng 8g curcumin mỗi ngày mà vẫn an toàn cho sức khỏe. Những bệnh nhân có hiện tượng tăng sản tế bào tuyến tiền liệt hay trong tiền sử gia đình có trường hợp bị ung thư vú thì nên sử dụng để phòng ngừa mỗi ngày.

    Tuy nhiên để có nhưng cơ sở khoa học của curcumin trong điều trị vẫn cần có những nghiên cứu sâu hơn để khẳng định lại cơ chế tác dụng của curcumin lên các dạng ung thư cũng như cần thực hiện thêm các thử nghiệm lâm sàng.
